PRIVACY POLICY
 
The following sets out the Privacy Policy for Paula Sladdin including its website and and booking services. Paula Sladdin is bound by the provisions of the Commonwealth Privacy Act 1988 and the Health Privacy Principles in the Victorian Health Records Act 2001 and all other State and Commonwealth legislation concerning the privacy of individuals.
Paula Sladdin will only transfer health information outside Victoria if the receiving organisation is subject to laws substantially similar to the Health Privacy Principles, or confidentiality and disclosure agreements are in place between the individual and the external organisation.
 
COLLECTION OF PERSONAL INFORMATION
Many aspects of the services of Paula Sladdin require you to submit personally identifiable information – that is, information about an individual, whose identity is apparent or can be reasonably ascertained from the information.  This includes information collected by through the clinics, over the phone and email, social media, from the website and practice management system.
 
SECURITY
Paula Sladdin’s goal is to manage your personal information confidentially and securely. Paula Sladdin will safeguard the personal information it holds against interference, misuse, loss, unauthorized access and modification.  Paula Sladdin ensures that any providers of IT services to us (including overseas providers of IT services including Cloud services) are also privacy compliant. Paula Sladdin will monitor and implement appropriate technical advances or management processes, to safeguard personal information.
Personal information that is no longer required will be destroyed or deleted in accordance with Health Privacy Principle 4. In many circumstances, however if you consent in writing, it will be kept for marketing purposes.
Paula Sladdin will take all reasonable steps to ensure that the data collected, used or disclosed is accurate, complete and up to date, and has been obtained directly from relevant individuals or other reputable sources.
 
SHARING OF YOUR PERSONAL INFORMATION
If sensitive information is collected (as defined in s 6 of the Privacy Act), the information will be treated with security and confidentiality. Your information will not be sold to other organisations.
Paula Sladdin will only disclose personal information in accordance with the Privacy Act.  This means that personal information may be disclosed:
  • for the purposes for which we have advised that we are collecting it, and for related purposes that the individual would reasonably expect;
  • where we have the consent of the individual to do so;
  • as required by law; or
  • under other circumstances where permitted under the Privacy Act.
In the course of business activities, Paula Sladdin may need to disclose some of your personal information to relevant staff.  Paula Sladdin may occasionally hire other companies to provide services on our behalf, including, but not limited to handling customer support enquiries, processing transactions or customer freight shipping.  Those companies will be permitted to obtain only the personal information they need to deliver the service. Paula Sladdin takes reasonable steps to ensure that these organisations are bound by confidentiality and privacy obligations in relation to your personal information.
From time to time, Paula Sladdin may use customer information for new, unanticipated uses not previously disclosed in our Privacy Policy.  If Paula Sladdin’s customer information practices change, data collected from the time of the policy change forward will adhere to our updated practices.
Where an individual chooses not to provide requested information, we will advise that individual of the effect this non-disclosure may have. For example, withholding certain information may limit the ability to provide relevant services to individuals. Paula Sladdin may choose not to provide a service if certain important information is not permitted to be collected.
 
CHANGES TO THIS PRIVACY POLICY
This Privacy Policy will be reviewed from time to time and any amendments will be incorporated into the updated policy.
 
ACCESS TO PERSONAL INFORMATION
You have a right to access your personal information and this right extends to correction of the information if it is inaccurate, incomplete, misleading or not up to date. If you would like to do so, please let us know. You may be required to put your request in writing for security reasons. Paula Sladdin reserves the right to charge a fee for searching, and providing access to, your information on a per request basis.
In some circumstances allowed by the Privacy Act, Paula Sladdin will be entitled to refuse access to information and if we do so, we will consider whether a mutually agreed intermediary will allow sufficient access to meet your needs and ours.
Paula Sladdin is committed to protecting the privacy of individuals and will view unauthorised disclosure of, or access to, personal information by our employees or contractors, as a serious breach of this policy. Appropriate action (which may include disciplinary or legal action) will be taken in such cases.
 
COOKIES
Cookies are small digital signature files that are stored by your web browser that allow your preferences to be recorded when visiting the website. Also they may be used to track your return visits to the website.  Third party advertising companies may also use cookies for tracking purposes. When you use our website, having your cookies enabled will allow us to maintain the continuity of your browsing session and remember your details when you return. We may also use web beacons, Flash local stored objects and JavaScript. If you adjust your browser settings to block, reject or delete these functions, the webpage may not function in an optimal manner. We may also collect information about your IP address, although this may not identify you.
For each visitor to the website, we expressively collect the following non-personally identifiable information, including but not limited to browser type, version and language, operating system, pages viewed while browsing the site, page access times and referring website address. This collected information will be used solely for the purpose of gauging visitor traffic, trends and delivering personalised content to you while you are at this site.
 
OPT-OUT
We will always provide individuals with a zero-cost way of contacting us to register a request to “opt -out” from receiving any product offers.
 
CREDIT CARD INFORMATION
Paula Sladdin uses NAB to process all payments including credit card payments. Any credit card information that is provided over the phone for payments will not be kept on file unless authorised by you in writing. Credit card information is stored under password encryption and can only be accessed by our authorised staff members.
